Amends Connecticut's residential property condition disclosure form (section 20-327b) to require sellers to disclose whether they are aware of any dam on the property that has been or is required to be registered with the Department of Energy and Environmental Protection, effective October 1, 2021

Adds a new "Important Information" section advising prospective buyers that information concerning dam registration and categorization may be obtained from DEEP

Renumbers existing disclosure questions in the "General Information" subsection to accommodate the new dam disclosure requirement

Maintains all other existing disclosure requirements covering property conditions including flood hazard areas, wetlands, underground storage tanks, foundation issues, and pyrrhotite presence
