Establishes a program through the Commissioner of Public Health providing three-year grants to community-based primary care providers to expand healthcare access for the uninsured, supporting direct services, clinician recruitment/retention, and capital expenditures.

Expands the definition of "primary care clinicians" to include psychiatrists, psychologists, licensed clinical social workers, licensed marriage and family therapists, and licensed professional counselors.

Transfers responsibility for the community health worker grant program from the Department of Public Health to the Department of Social Services, extending the grant availability period through June 30, 2024, with a maximum of $30,000 annually per community health worker and $6 million total aggregate.

Allocates $3,000,000 in federal funds for fiscal years 2022 and 2023 to support the community health worker program, which provides grants to community action agencies serving COVID-19 affected populations.

Increases the minimum per diem rate to $501 per bed for residential facilities providing intermediate care for individuals with intellectual disabilities under the Medicaid program for fiscal years 2022 and 2023.
