Changes the fiscal year threshold for Tobacco Settlement Fund disbursements from 2018 to 2023, with transfers to General Fund and remainder to Tobacco and Health Trust Fund.

Establishes the Tobacco and Health Trust Fund as a nonlapsing fund to support programs reducing tobacco abuse through prevention, education, and cessation, as well as programs addressing substance abuse and unmet physical and mental health needs.

Increases the maximum annual disbursement limit from the Tobacco and Health Trust Fund from six million dollars to twelve million dollars per fiscal year for eligible programs, effective for fiscal year ending June 30, 2023 and thereafter.

Maintains board of trustees governance structure consisting of seventeen members appointed by the Governor, legislative leadership, and Secretary of Office of Policy and Management, with priority given to programs addressing tobacco and substance abuse serving minors, pregnant women, and parents of young children.

Requires board to submit annual activity reports by January 1 and comprehensive performance evaluation reports by February 1 to joint standing committees of the General Assembly on public health and appropriations.
