Requires proposed new owners of health care facilities to submit change of ownership applications at least 120 days before the proposed ownership change, including detailed financial information, organizational charts, compliance history, and Medicare/Medicaid provider status.

Department of Public Health must schedule an inspection to verify facility compliance with state requirements; commissioner may deny the application or require a consent order with corrective action plan if violations are found.

Expands the definition of ownership changes subject to approval to include any change in beneficial ownership of the entity owning the facility, not just 10 percent or more of corporate stock.

Permits changes in legal form of ownership entity (e.g., corporation to limited liability company) without approval if beneficial ownership remains unchanged and owner provides required information to the department.

Adds notice requirement on nursing home licensure applications stating that licensees, owners, officers, and administrators may face civil, criminal, and administrative liability for abuse or neglect of residents perpetrated by facility employees.
