Commissioner of Social Services shall design and implement a Medicaid reimbursement program for certified community health workers providing services to HUSKY Health program members.

Covered services include coordination of medical, oral and behavioral health care services and social supports; connection to and navigation of health systems; prenatal, birth, lactation and postpartum supports; and health promotion and self-management education.

Reimbursement rates must support workforce growth and not incentivize other providers to limit access to community health workers.

Commissioner must consult with certified community health workers, Medicaid beneficiaries, disability advocates, and others to ensure the program includes both community-based and clinic-based workers and reflects medical assistance program member demographics.

Commissioner must submit annual reports to the joint standing committee on human services and the Council on Medical Assistance Program Oversight beginning January 1, 2024, until full program implementation, evaluating impact on health outcomes and health equity.
