Revises Medicaid reimbursement rates for community living arrangements, intermediate care facilities for individuals with intellectual disabilities, residential care homes, and nursing facilities, effective July 1, 2023.

Establishes an acuity-based methodology for nursing home reimbursement effective July 1, 2022, with case-mix adjustments based on Minimum Data Set resident assessment data and cost data from September 30, 2019.

Divides allowable nursing home costs into five components: direct care, indirect care, fair rent, capital-related costs, and administrative/general costs, with maximum cost limits set at specified percentages of median allowable costs by peer grouping.

Prohibits inflation-based rate increases for fiscal years ending June 30, 2022 and 2023, but allows subsequent inflation adjustments calculated using the gross domestic product deflator applied prior to other budget adjustment factors.

Requires cost rebasing no more frequently than every two years and no less frequently than every four years beginning July 1, 2025, with no inflation adjustment during years when facilities' rates are rebased.
