Repeals and replaces Section 17a-227 to require Department of Developmental Services licensure for community living arrangements and community companion homes serving persons with intellectual disability, Prader-Willi syndrome, or autism spectrum disorder.

Establishes regulatory requirements including cardiopulmonary resuscitation certification for staff, staffing schedule records available for inspection, emergency planning and training, and staff verification of emergency preparedness during quality reviews and licensing inspections.

Requires all residential facilities serving persons with Down syndrome age 50 or older to have at least one staff member trained in Alzheimer's disease and dementia care, and mandates unannounced licensure-related visits for community living arrangements.

Mandates department contracts with licensed providers include provisions for periodic performance reviews and progressive enforcement actions for poor performance, including strict monitoring schedules, partial-year contracts, and payment reductions until compliance is achieved.

Authorizes the Commissioner to disclose names of licensed operators whose licenses were revoked or surrendered due to abuse or neglect findings to authorized agencies, employers, and other state departments for protective service and employment determinations.
