Allows applicants with unforeseen illness or physical disability occurring within six days before an election to appoint a designee to deliver their absentee ballot.

Extends this designee appointment option to patients in hospitals or nursing homes within the six-day period before an election, primary, or referendum.

Requires the designee appointment to include the applicant's statement of the illness/disability date or the name and address of the hospital/nursing home, plus the designee's name, address, and category.

Requires the designated person to sign a statement consenting to deliver the ballot without tampering with it in any way.

Allows the designated person to personally submit the application to the municipal clerk within six days before the election, at which point the clerk shall provide the absentee voting set.
