Establishes Connecticut Housing Engagement and Support Services (CHESS), a home and community-based Medicaid program under federal Sections 1915(i) and 1915(b)(4) to improve housing stability and health outcomes for persons experiencing or at risk of homelessness, effective July 1, 2024.

Requires the Commissioner of Social Services, in consultation with the Commissioners of Housing and Mental Health and Addiction Services, to develop a strategic plan addressing CHESS program outcomes, streamlined multiagency administrative procedures, and additional Medicaid waiver programs targeting social determinants of health including housing, nutrition, and health management.

Mandates a report to relevant joint standing committees by January 1, 2025, including metrics on participant health, housing and economic stability, progress on streamlining procedures, identification of additional Medicaid programs, and application timelines.
