Requires each registrar of vital statistics to issue both standard death certificates and short form death certificates

Short form death certificates must include: decedent's name, date of death, town of death, date of birth, and Social Security number

Restricts access to short form death certificates containing the Social Security number to only surviving spouses or next of kin

Amends Chapter 93 of the Connecticut general statutes governing vital statistics
