Requires payers to provide opt-in forms to employers of self-funded employee health plans within 15 days of retaining claim administration services, allowing employers to voluntarily submit their health plan data for state health care expenditure calculations

Establishes annual reporting requirements by January 31, 2026 for payers to submit lists of employers who opted in or out of data submission, along with officer certifications confirming reasonable efforts to distribute opt-in forms

Authorizes penalties of up to $10 per covered individual enrolled in a self-funded plan against payers who fail to provide the required opt-in forms to employers

Prohibits payers from charging employers fees beyond actual costs incurred for submitting self-funded data to the commissioner

Effective October 1, 2025, amends sections 19a-754h and 19a-754j(a) of Connecticut general statutes governing health care cost growth benchmark program data collection and public hearings
