Commissioner of Public Health must establish a Triple Bottom Line Justice (TBLJ) demonstration pilot program by January 1, 2027, in a city with population over 148,000, contracting with a community-led redevelopment organization to administer it.

Program focuses on "Brownfields to Healthfields" (B2H) methodology to transform contaminated properties in areas with pollution, disease, poverty, and violence into community-strengthening reuses.

Commissioners must identify ICD-10 Z codes relevant to environmental and climate stressors and develop a plan to increase utilization rate to at least 20% of all health insurance claims by January 1, 2030.

Annual reports required from January 1, 2027 through 2030 to legislative committees on public health, environment, and insurance, covering Z code utilization rates, health trends, and recommendations.

Final report on pilot program findings due January 1, 2028, with program terminating on that date or when report is issued, whichever is later; effective date October 1, 2026.
