Expands the Department of Public Health's change of ownership approval process for health care facilities to include review of real property owners and beneficial owners of real property, not just facility operators

Requires disclosure of the name and address of any licensed health care facility owned, operated, or managed by proposed new real property owners during the preceding five years, along with compliance history

Authorizes the Commissioner to waive real property owner disclosure requirements if the proposed owner demonstrates the real property owner will have no involvement in nursing home management or operations

Commissioner may consider lease terms, the property owner's lack of nursing home ownership/management history, and other relevant information when determining whether to grant such waivers
