Defines "high-intensity endurance or power sport" as football, basketball, soccer, hockey, lacrosse, and middle/long-distance track events

Requires the State Board of Education to develop a sudden cardiac arrest screening form by July 1, 2027 for students participating in high-intensity sports

Screening form must ask about student's history of chest pain with exertion, unexplained fainting, sudden cardiac arrest, or family history of sudden cardiac arrest/death

Students who report cardiac risk factors on the screening form must receive an electrocardiogram test and written medical clearance before participating in high-intensity sports

Coaches of high-intensity sports must distribute the screening form to parents/guardians and obtain completed, signed forms before each season, effective July 1, 2027
