Requires hospitals to identify nutrition needs of community members with diabetes and congestive heart failure when conducting community health needs assessments

Mandates hospitals include identified nutrition needs in their community health needs assessment documentation, to the extent permissible under federal law

Requires hospitals to incorporate medically tailored and food-based interventions for diabetes and congestive heart failure patients in their implementation strategies

Amends Section 19a-127k of the general statutes by adding new subsection (j)

Effective date: October 1, 2026
