Commissioner of Social Services must rebalance the strategic plan for Medicaid long-term care beginning July 1, 2026 to increase home and community-based care options for Medicaid beneficiaries and persons with income up to 300% of the federal poverty level

Department of Social Services required to conduct increased outreach to eligible individuals for the Connecticut home-care program for the elderly and Medicaid waiver programs, and assist with application completion

Long-Term Care Planning Committee must study systems to expand long-term care options allowing more beneficiaries to receive care at home rather than in skilled nursing facilities

Committee must submit report by December 1, 2026 to legislative committees on aging, appropriations, human services, and public health with recommendations and estimated state savings from reduced skilled nursing facility care

Strategic plan development must include home, community-based, and institutional providers and consider regional aging trends, service gaps, care quality, facility conditions, bed supply, and rate structures
