Reauthorizes the Clean Air Act Title V Operating Permit Program through December 31, 2014, extending the program that oversees the state's largest air emission sources.

Maintains permit fee levels for 2012-2014 at the same rates charged during 2008-2011, with fees ranging from $2,000 to $243,000 depending on emission source category.

Establishes 11 operating permit categories based on operational hours and 9 categories based on annual emissions tonnage, with new sources applying after January 1, 2011 paying fixed fees.

Requires the Department to consult with the Title V Operating Permit Program Advisory Committee before increasing full-time equivalent employees funded by the program.

Imposes 2% monthly compounding interest on delinquent fees and authorizes permit cancellation for sources delinquent over one year; requires independent third-party review of the program funded outside the fee structure.
