Renames Chapter 35, Title 31 of the Delaware Code from "Elder Care Information And Support System For Delaware Families" to "Delaware Aging and Disability Resource Center"

Establishes the Delaware Aging and Disability Resource Center within the Division of Services for Aging and Adults with Physical Disabilities of the Department of Health and Social Services

Center responsibilities include operating statewide to provide information, awareness, options counseling, and streamlined access to long-term care services for elderly and adults with physical disabilities

Center must provide hospital discharge planning supports, plan for long-term expansion and sustainability, and conduct quality assurance and evaluation activities

Combines existing Alzheimer's-specific information into the program and supports future federal grant applications for Aging and Disability Resource Center projects
