Establishes the Delaware Prescription Monitoring Program (PMP) administered by the Office of Controlled Substances to monitor prescribing and dispensing of Schedule II, III, IV, and V controlled substances and drugs of concern.

Requires dispensers to submit prescription data including patient name, date of birth, prescriber information, drug details, quantity, and date dispensed to the PMP database.

Allows prescribers to access patient utilization reports from the preceding 12 months before writing controlled substance prescriptions when there is reasonable belief the patient may be seeking drugs for non-medical purposes.

Restricts access to prescription monitoring information to authorized parties including prescribers, law enforcement, regulatory boards, Medicaid officials, and grand juries, with strict confidentiality protections and criminal penalties for unlawful disclosure (Class G felony) or using information to further crimes (Class E felony).

Requires the Office of Controlled Substances to submit annual reports to the General Assembly evaluating the program's cost-benefits and impact on controlled substance diversion and abuse.
