Establishes a Heart Disease and Stroke Prevention Program within the Division of Public Health to address cardiovascular disease and stroke-related health issues in Delaware.

Program purposes include conducting health education and public awareness activities, coordinating prevention strategies, providing resources on cardiovascular disease and stroke data, and improving access to treatment for underinsured and rural populations.

Requires the Department of Health and Social Services to maintain aggregate statewide stroke data using nationally recognized, evidence-based guidelines aligned with American Heart Association/American Stroke Association and Centers for Disease Control and Prevention standards.

Mandates the Department make heart disease and stroke incidence, morbidity, and mortality data publicly available by census tract and create color-coded annual maps for each Delaware county showing disease incidence levels.

Becomes effective upon appropriation by the General Assembly of sufficient funds to implement the program's provisions.
