Establishes a Heart Disease and Stroke Prevention Program within the Division of Public Health of the Department of Health and Social Services to address cardiovascular disease and stroke-related health issues through education, prevention, and community outreach.

Requires the Department to maintain aggregate statewide stroke data using nationally recognized guidelines from the American Heart Association/American Stroke Association, CDC, and Joint Commission, with information made available to the public on the agency's website.

Mandates the Department create and annually update detailed maps for each Delaware county showing heart disease and stroke incidence by census tract, color-coded to designate disease incidence levels, and post maps publicly on its website.

Requires the Department to make heart disease and stroke incidence, morbidity, and mortality data available as public records by census tract, with data combined across contiguous tracts only when necessary to protect patient confidentiality.

Takes effect upon appropriation by the General Assembly of sufficient funds to implement the program's provisions.
