Establishes a State Transition Task Force for Emerging Adults with Disabilities and Special Health Care Needs, to be created and supported by the Governor's Advisory Council for Exceptional Citizens (GACEC) with research support from the Center for Disabilities Studies at the University of Delaware and the Catalyst Center at Boston University School of Public Health.

Task Force consists of 21 members including state agency representatives (Labor, Education, Health and Social Services, etc.), medical professionals, disability advocates, three young adults with disabilities, three family members, and legislative appointees.

Creates four subcommittee work groups to address housing/transportation, health care, education, and employment for young adults with disabilities and special health care needs.

Task Force shall assess transitional service needs, identify barriers to access, develop a statewide transition services plan accommodating geographic and cultural diversity, establish performance measures, and identify funding sources.

Task Force must submit a final report with findings and recommendations to the Governor, Senate President pro tempore, and House Speaker within one year of passage.
