Extends the Title V Operating Permit Program fee collection authority from calendar years 2012-2014 to 2015-2017, with an additional review period through 2020.

Revises annual base fees and user fees for sources based on operating hours and emissions, with new fee schedules ranging from $5,700 to $277,020 for base fees and $3,950 to $350,000 for user fees depending on source category.

Establishes a $300,000 annual fee credit program for 2015-2017 distributed to existing Title V and synthetic minor permit holders without delinquent accounts, based on their 2014 categorical status.

Strengthens permit enforcement provisions by allowing the Department to revoke Title V permits solely for nonpayment of annual fees after providing 60 days' written notice or when fees are three months overdue.

Requires an independent third-party management system review of the Title V Program by December 31, 2015, funded up to $100,000 from existing fee balances, to optimize Division of Air Quality operations and fee structures.
