Updates Delaware's advance health-care directive statute to reflect the January 2011 transfer of the Office of the Long Term Care Ombudsman from the Division of Services for Aging and Adults with Physical Disabilities (DSAAPD) to the Delaware Health and Social Services Office of the Secretary

Replaces references to "Division of Services for Aging and Adults with Physical Disabilities" with "Delaware Health and Social Services" throughout Title 16, §2505 and §2511

Maintains the requirement that advance health-care directives executed by residents of nursing homes, rest homes, or similar long-term care facilities must have a patient advocate or ombudsman designated by the state agency as one of the witnesses

Preserves oversight authority over advance health-care directives for long-term care facility residents, transferring this responsibility to Delaware Health and Social Services
