Amends Delaware Code Title 10, Section 5901 to permit individuals under supervision of the Delaware Department of Correction to petition for name changes based on sincerely held religious beliefs or gender identity.

Court of Common Pleas may grant name change petitions when testimony or sworn affidavits establish the petition is motivated by sincere religious belief or gender identity.

Requires courts to provide the Delaware Department of Correction with notice and opportunity to oppose proposed name changes and submit supporting documentation.

Maintains the abrogation of common law name change rights for individuals subject to Department of Correction supervision, requiring all name changes to proceed through court petition process.
