Establishes Family Court jurisdiction over youths ages 18-26 with severe and persistent mental or behavioral health disorders who were under DSCYF custody when turning 18 and remain mentally ill.

Allows petitions for jurisdiction to be filed when youth reaches 17½ years old but no later than 6 months before 21st birthday; jurisdiction terminates at age 26 or by court order.

Requires annual review hearings where court evaluates youth's financial stability, housing, medical benefits, employment, education, community connections, and mental health care.

Permits Family Court to order youths to participate in services or outpatient treatment, with involuntary treatment requiring findings under Title 16 § 5013; noncompliance triggers priority review hearings.

Clarifies DSCYF is not required to provide foster care payments or stipends to youths under this jurisdiction and does not affect age of majority or automatic termination of custody at age 18.
