Establishes the Delaware Medical Orders for Scope of Treatment (DMOST) Act as a new Chapter 25A in Title 16, creating a standardized, portable medical document for patients with serious illness or frailty to express end-of-life care preferences.

Requires DMOST forms to be voluntarily completed and signed by patients (or authorized representatives if lacking decision-making capacity) with health-care practitioner signature, and permits modification or revocation at any time by patients with capacity.

Mandates health-care providers, institutions, and emergency-care providers comply with DMOST directives regarding scope of treatment and life-sustaining interventions, while providing immunity for good-faith compliance.

Authorizes the Department of Health and Social Services to promulgate regulations, develop standardized DMOST forms, and establish training requirements; allows Delaware Health Information Network to create electronic registry for DMOST forms.

Establishes civil penalties up to $1,000 per violation for non-compliant institutions and criminal penalties including class G felony for falsifying, forging, or coercing DMOST forms, and class A misdemeanor for requiring DMOST completion as condition of insurance or care.
