Requires the Affidavit of Religious Belief for vaccination exemptions to include acknowledgment that children may be temporarily excluded from school during vaccine-preventable disease outbreaks.

Adds language stating parents must be informed by schools about provisions for temporary exclusion during outbreaks and that they acknowledge responsibility for helping their child keep up with schoolwork during exclusion.

Makes medical exemptions subject to review and approval by the Division of Public Health, and clarifies that clinical illness history of measles or rubella cannot be used as exemption justification.

Changes the trigger for temporary school exclusion from an "epidemic throughout the State or in a particular region" to an "outbreak" as determined by the Division of Public Health, or when a child has had or is at risk of exposure to a vaccine-preventable disease.

Requires approval by the Division of Public Health for a medically exempt student to return to school, rather than automatic return upon lifting of an epidemic declaration.
