Extends rabies vaccination requirements to ferrets and adds exemptions for dogs, cats, and ferrets when a licensed veterinarian certifies that vaccination would endanger the animal's health due to age, infirmity, disability, illness, or other medical considerations, provided a titer test is administered.

Requires veterinarians to maintain valid rabies vaccination certificates or exemption certificates for kennel dogs for 12 months after expiration and to provide certificates to new owners when dogs are sold or traded.

Mandates that licensed veterinarians complete and review a Vaccine Risk Assessment Form with pet owners prior to administering vaccines, assessing vaccine risks on a 1-5 point scale based on animal health and disease exposure risk.

Requires the Vaccine Risk Assessment Form to include discussion of titer tests as an alternative to unnecessary vaccinations and spacing of vaccines over time to avoid medical problems; original forms must be retained for 3 years.

Designates the act as the "Maggie's Pet Vaccine Protection Act" and requires the Department of Health and Social Services to develop and distribute the Vaccine Risk Assessment Form to veterinarians by January 1, 2019.
