Requires mortgagees to file a statement of mortgagee address change within 60 days of changing their notice address with the recorder of deeds office in the county where the mortgage is recorded.

Establishes a penalty of up to $1,000 fine plus up to $1,000 in costs for failure to file a statement of mortgagee address change, with Superior Court jurisdiction over violations.

Mandates each recorder of deeds create and maintain a separate "statement of mortgagee address change index" that references the mortgagee and their new notice address, but does not require listing each individual mortgage.

Allows each recorder of deeds to promulgate a sample form for filing address changes and to charge fees that reasonably reflect the costs of creating and maintaining the address change index.

Clarifies that mortgagees need only file one address change statement per address change rather than separate statements for each affected mortgage.
