Expands the Primary Care Reform Collaborative membership to include representatives from the Medical Society of Delaware, Delaware Nurses Association, Delaware Healthcare Association, insurance carriers, employers, and Federally Qualified Health Centers.

Creates the Office of Value-Based Health Care Delivery within the Department of Insurance to reduce health care costs by promoting high quality, cost-efficient health insurance products with stable and affordable rates.

Requires the Office to establish affordability standards for health insurance premiums based on Primary Care Reform Collaborative recommendations and set targets for carrier investment in primary care by January 1, 2025.

Directs the Office to collect data on carrier investments and primary care spending, monitor compliance with reimbursement rate requirements, and make annual recommendations to the Insurance Commissioner and Collaborative.

Mandates the Insurance Commissioner adopt initial policies and procedures by January 1, 2020, with full implementation of the Office by April 1, 2020.
