Requires physicians to offer patients ultrasound imaging and auscultation of fetal heart tone services before terminating a pregnancy, except in medical emergencies.

Ultrasound offer must include active ultrasound image showing fetal dimensions and internal/external organs, and auscultation of fetal heart tone, both of which the patient may choose to view or hear.

Patients must sign a form confirming they were offered ultrasound and auscultation services before abortion is performed.

Establishes criminal penalties of class A misdemeanor for physicians who intentionally or knowingly fail to comply with requirements, with exception for good faith belief of compliance.

Provides civil remedies including medical malpractice actions, professional disciplinary action, and wrongful death claims; allows plaintiffs to use pseudonyms and recover attorney's fees if judgment is rendered in their favor.
