Establishes a process for the Department of Health and Social Services, Department of Correction, Department of Safety and Homeland Security, and Department of Services for Children, Youth and Their Families to link and share criminal violence data between agencies.

Directs these agencies to create and implement a criminal violence risk assessment tool based on recommendations from a November 2015 CDC report on firearm violence in Wilmington, Delaware.

Requires the risk assessment tool to be suitable for assessing both juveniles and adults and to be usable by the Department of Health and Social Services, Department of Correction, Division of Youth Rehabilitation Services, and courts.

Mandates use of risk assessment reports by courts for pretrial release and sentencing decisions, by the Department of Correction for probation and parole programs, and by the Board of Pardons or Board of Parole when reviewing applications.

Authorizes the General Assembly to appropriate funds annually to implement the chapter and permits the four state agencies to cooperatively adopt regulations for implementation.
