Encourages Delaware to become a "suspension versus termination" state for Medicaid coverage, maintaining health insurance for individuals released from correctional facilities rather than terminating it upon incarceration.

Directs the Division of Medicaid and Medical Assistance and Department of Correction to collaborate on a reentry process that seamlessly connects individuals with opioid use disorder treatment and other necessary services upon release.

Requires eligible individuals without coverage to apply for Medicaid benefits prior to release, including selecting a managed care organization and providing consent for information sharing between the Department of Correction, Medicaid, and healthcare providers.

Mandates the Department of Correction develop a definition and process for identifying individuals with high health needs, including those with opioid use disorder and other substance use disorders.

Requires managed care organizations to be notified before individuals are released and to contact high-needs individuals prior to release to develop personalized care plans with evidence-based treatment options and community service referrals.
