Adds the Department of Correction and contracted third-party health care providers to mandatory reporting entities required to participate in the Delaware Health Information Network (DHIN) under Title 16.

Requires the Department of Correction or third-party medical care providers to enroll in DHIN as active users of the Community Health Record no later than 30 days after beginning to provide medical care to incarcerated individuals.

Requires the Department of Correction or third-party entities to enter into an agreement with DHIN to provide summaries of each visit or episode of care in an electronic format no later than 90 days after beginning to provide medical care.

Applies enrollment and agreement requirements to entities already providing medical care to incarcerated individuals as of the enactment date, with compliance deadlines of 30 and 90 days respectively after enactment.

Aims to improve care coordination and data sharing for individuals in the Department of Correction's custody by ensuring health data and costs are included in DHIN's clinical health data repository and Delaware Health Care Claims Database.
