Establishes Chapter 30P in Title 16 of Delaware Code to implement recommendations from the CDC's November 2015 report on firearm violence in Wilmington.

Requires the Departments of Health and Social Services, Correction, Safety and Homeland Security, and Services for Children, Youth and Their Families to create a data linking and sharing process to coordinate services for individuals at highest risk of violence involvement.

Directs these agencies to construct and implement a criminal violence risk assessment tool suitable for evaluating both juveniles and adults for use by courts, corrections, youth rehabilitation services, and social service agencies.

Specifies the risk assessment tool reports must be used as an additional tool by courts for pretrial release and sentencing decisions, by corrections for probation and parole administration, and by the Board of Pardons or Parole when considering applications.

Authorizes the General Assembly to annually appropriate funds necessary to implement the chapter and allows participating agencies to cooperatively adopt implementing regulations.
