Adds new captive insurer fees: $3,200 processing fee for examining and processing initial applications and $100 application fee for conditional certificate of authority

Creates registration fees of $1,000 for both initial registration and annual continuation for reciprocal jurisdiction reinsurers and certified reinsurers

Updates fraternal benefit society statutes to reference the centralized fee schedule in § 701 rather than specifying dollar amounts ($25) directly in individual code sections
