Requires all individual health insurance policies delivered, issued, or renewed after December 31, 2024, to cover annual prostate screening at no cost for men over 40 at high risk of prostate cancer.

Defines high-risk individuals as African American men over 40 and men over 40 with a family history of prostate cancer.

Amends group and blanket health insurance policies to provide prostate-specific antigen (PSA) test coverage as an annual screening for men over 50, African American men over 40, and men over 40 with a family history of prostate cancer.

Exempts accident-only, specified disease, hospital indemnity, Medicare supplement, long-term care, disability income, and limited benefit plans from coverage requirements.

Takes effect January 1 following enactment into law.
