Eliminates all cost-sharing (deductibles and coinsurance) for medically necessary physical therapy, occupational therapy, and neuromuscular massage therapy services across individual health insurance policies, group policies, state employee health plans, and Medicaid.

Expands prohibition on annual or lifetime numerical limits on physical therapy, occupational therapy, and neuromuscular massage therapy to cover treatment of any chronic or acute musculoskeletal pain or post-surgical therapy, rather than limiting the prohibition to back pain treatment.

Applies to all policies issued, renewed, modified, or reissued after December 31, 2025, with the act expiring on December 31, 2030 unless extended by the General Assembly.

Exempts accident-only, specified disease, hospital indemnity, Medicare supplement, long-term care, disability income, and limited benefit health insurance policies from the cost-sharing elimination requirement.

Permits high deductible health plans and catastrophic plans to limit cost-sharing only after deductibles are met to maintain federal tax treatment eligibility.
