Adds definitions for "collaborative pharmacy practice" and "collaborative pharmacy practice agreement" to allow pharmacists to provide patient care and drug therapy management services under written agreements with practitioners.

Expands the definition of "practice of pharmacy" to include collaborative pharmacy practice as authorized by collaborative pharmacy practice agreements.

Modifies immunization administration requirements to allow pharmacists, pharmacy interns, and nationally certified pharmacy technicians (who are CPR-trained and have notified the Delaware Board of Pharmacy) to administer immunizations to patients 3 years of age and older under prescriber order or protocol.

Requires pharmacists to report all vaccine administrations to DELVAX within 72 hours, including patient name, vaccine name, injection site, lot and expiration numbers, administering facility, and date of administration.

Updates the definition of "practitioner" or "prescriber" to include individuals authorized to prescribe drugs through collaborative pharmacy practice agreements.
