Creates a DMOST Program within the Department of Health and Social Services to improve utilization of Delaware Medical Orders for Scope of Treatment forms through education, training, and outreach to healthcare practitioners, providers, emergency responders, and the public.

Establishes a DMOST Steering Committee comprised of representatives from healthcare organizations, insurance, the Long-Term Care Ombudsperson, Delaware State Bar Association, Delaware Quality of Life Coalition, and public members to evaluate and improve the DMOST program.

Requires the DMOST Steering Committee to produce an annual report including data on the number of forms in the electronic registry, registry consultation frequency, emergency care provider use, training information, public education efforts, and recommendations for program improvement.

Directs the Department to work with the Delaware Health Information Network to maintain the electronic DMOST registry and coordinate with the National POLST Coalition regarding best practices and research.

Clarifies the Department's authority to develop protocols for DMOST form completion, modification, and revocation, and to impose civil monetary penalties for violations.
