Amends the Health Care Provider Loan Repayment Program (HCPLRP) to award education loan repayment grants of up to $50,000 per year for a maximum of 4 years to qualifying clinicians.

Expands eligible practice sites to include hospital primary care practices, private practices, federally-qualified health centers, community outpatient facilities, community mental health facilities, free medical clinics, and school-based health centers located in underserved areas.

Requires private practice sites and eligible practitioners to participate in the Voluntary Initiative Program administered by the Department of Health and Social Services' Health Care Connection, and requires hospital sites to provide dollar-for-dollar matching funds.

Extends eligibility to new primary care and dental providers (including dentists, nurse practitioners, psychologists, and clinical social workers) who completed graduate education within 2 years of applying, practicing in ambulatory and outpatient settings.

Authorizes the Commission to prioritize applications from DIMER or DIDER-participating students in Delaware-based residency programs and spend up to $150,000 annually on marketing to attract clinicians to the program.
