Mandates that facility ownership information be posted on the facility's website and included in all advertising, marketing, and promotional materials

Requires the Department of Health to publish current ownership information for all long-term care facilities on its website

Requires persons assuming ownership of a long-term care facility to notify current residents and their representatives at least 60 days before the transfer, including information about the new owner's other facilities and any enforcement actions against them

Authorizes the Department to delay or deny licensure to prospective owners who have a pattern of serious violations or enforcement actions at other facilities they own or operate
