Creates a new crime of "application insurance fraud" targeting false, incomplete, or misleading statements made on insurance applications or renewal documents with intent to defraud insurers

Classifies application insurance fraud as a class G felony when the value of property, services, or benefits wrongfully obtained is $1,000 or more; otherwise classified as a class A misdemeanor

Each act of fraud constitutes a separate offense, with multiple fraudulent statements in a single application each counting as distinct violations; five or more acts may be aggregated for liability purposes

Establishes that signing or initialing an insurance application may create an inference that the person read and reviewed the document

Defines "insurer" broadly to include health service corporations, health maintenance organizations, and managed care organizations
