Prohibits health insurers from requiring contracted health care practitioners to accept terms of other practitioner contracts with the insurer or affiliated entities, except for practitioners in group practices who must accept group-negotiated terms.

Prevents health insurers, prepaid limited health service organizations, and health maintenance organizations from requiring practitioners to provide services at insurer-set fees unless those services are covered under the applicable health insurance policy or subscriber agreement.

Defines "covered services" as services reimbursable under the applicable health insurance policy or subscriber agreement, subject to any contractual benefit limitations.

Applies all fee-setting restrictions to contracts entered into or renewed on or after July 1, 2010.

Voids any contract provisions that violate these restrictions and exempts violations from criminal penalties under Florida law.
