Authorizes insurers to offer voluntary wellness programs with rewards including merchandise, gift cards, premium discounts, health savings account contributions, and modifications to copayments or deductibles, with medical verification allowed for nonparticipation due to health conditions.

Allows hospitals to appeal claim denials for failure to obtain necessary authorization due to unintentional error, requiring insurers to conduct retrospective medical necessity reviews within 30 business days and provide written clinical justification if denying the appeal.

Prohibits health insurers, prepaid limited health service organizations, and health maintenance organizations from requiring dentists to accept insurer-set fees for services unless those services are covered benefits under the contract.

Applies dentist fee restrictions to all contracts entered into or renewed on or after July 1, 2010.

Takes effect July 1, 2010.
