Adds optional $1 voluntary contributions on motor vehicle registration, driver license applications, and driver license renewals to fund homelessness prevention, with funds deposited into the Grants and Donations Trust Fund of the Department of Children and Family Services.

Creates a homelessness prevention grant program providing emergency financial assistance to families facing loss of housing due to financial or other crises, with maximum grants of $300,000 per lead agency and a goal that 85 percent of assisted families remain housed for one year.

Establishes that grant assistance may cover past due rent, mortgage payments, utility costs, case management services, and program administration costs not exceeding 3 percent of the grant award.

Limits lead agencies to spending a maximum of 8 percent of their funding on administrative costs and requires them to track and report on assisted families for at least 12 months after receiving assistance.

Repeals the existing emergency assistance program under section 414.16 and removes background check and substance abuse rehabilitation requirements from the Housing First approach to homelessness.
