Requires participating local government entities to submit a $500 nonrefundable application fee and conduct a competitive request for proposals before adopting an ordinance to participate.

Mandates participating entities reimburse the Department of Management Services for 100 percent of costs, including administrative costs of at least $2.61 per enrollee per month.

Establishes a 3-year minimum enrollment period with 1-year termination notice required; entities cannot reapply for 2 years after termination.

Prohibits participating entities from using the state's cafeteria plan for pretax premium contributions; instead, each entity must establish its own cafeteria plan with individual employee salary reduction agreements.
