Authorizes the Agency for Health Care Administration to seek a federal Medicaid waiver under section 1115 of the Social Security Act to implement a 5-year pilot project improving health care access in rural counties (defined as counties with fewer than 95,000 residents).

Requires contracts with health maintenance organizations to include community-based care infrastructure investments, medical home model implementation, hospital and pharmacy services, risk-adjusted rates, electronic health records development, and demonstrated cost savings.

Permits rural counties to voluntarily participate and develop single-source or regional requests for proposals for Medicaid services, and establishes local health advisory councils to review encounter data and process consumer complaints.

Allows rural county pharmacies to purchase drugs at federally qualified health center prices for Medicaid and Medicare Part D participants, with criteria developed in consultation with independent retail pharmacies and the Florida Pharmacy Association.

Requires the agency to submit a progress report to the Legislature by February 1, 2011, and takes effect July 1, 2010.
