Establishes an office within the Division of Children's Medical Services Network to oversee implementation of health care transition programs for adolescents and young adults (ages 12-26) with chronic physical, developmental, behavioral, or emotional conditions requiring specialized care.

Office responsibilities include developing planning and implementation processes, creating resource guides for providers, coordinating with training institutions, maintaining web-based materials, and disseminating evidence-based practices for health care transition services.

Department of Health must partner with Agency for Health Care Administration to seek federal waiver or state plan amendment addressing provider compensation strategies including co-management, care plan development, medical home provision, and telehealth technology to improve access to adult and specialty care.

Department of Health may contract with local health and transition services programs that include partnerships between adult and pediatric providers, community-based health care services, support organizations, and medical homes providing coordinated multidisciplinary care.

Requires at least one transition program in each of the eight regions of the Children's Medical Services Network and mandates independent evaluation of programs by qualified organizations or universities to measure process outcomes and client results.
